I have been in a lot of refineries and chemical plants. Most have safety as the highest priorities. Plants that process Liquefied compressed gases are the scariest. Most industrial accidents involve a series of several events. Liquefied gases when they escape, after vaporizing into a gas will cling to the ground and continue to spread until a spark is found no long series of events needed.
